You can download our Health history questionnaire here. Please take time to complete this and bring to your first visit.
Physician referral - Please contact your physician and let them know that you are seeking acupuncture as a way to address your concern. Referrals can be faxed to our office 734-975-2745. If you (or your doctor) have any questions regarding the referral process, please call the office and we’ll assist you. We are also happy to provide you with our office referral form to take to or fax to your physician, if more convenient.

Payment and Insurance Info.
- The Initial Office Visit is $120.00 and lasts 90 minutes. This appointment will include an in depth look at your health history and treatment of your condition.
- Follow up appointments are $75.00 and last 60 minutes. They include a brief consultation and an acupuncture treatment.
- Custom crafted herbal formulas are an additional $35.00.
- Our office accepts Visa and Mastercard personal checks and cash as payment.
Currently our office does not bill insurance companies. Many of our patients get reimbursement through medical flex accounts. We are happy to provide you with receipts to submit.

Our clinic, located on Ann Arbor’s East side, offers day, weekend and evening appointments to accommodate your busy schedule. Our classroom is the perfect venue to hold a variety of intimate lectures and classes. The entire office is wheelchair accessible and our onsite parking lot is convenient for those with mobility issues. We are easy to find, located on Carpenter Rd. near US23 & I94.
